Unfortunately, disease states usually provide us with little notice of their intention to "move in". Our Geriatricians have found that the only way to truly promote health is to evaluate and monitor the entire health and lifestyle of our patients. While our main focus may be to maintain a lower blood pressure, we will always be on the look out for the beginnings of other issues.

Our home care patients can expect a comprehensive initial evaluation. We will use this information to determine the level of care needed and to work in conjunction with our patients and their families to design a unique plan of care.

• Medication Review for Appropriateness in Seniors

• Liaison Services for Distant Families - Overseeing Care and Quickly Alerting Families to Problems

• Home Health Referrals and Orders for Necessary Treatments

• Hospice Counseling and Referrals

• Review of Financial, Legal or Medical Issues and Appropriate Referrals
